Vaccine Requirements for the School District of Philadelphia:
For attendance in all grades (K-12) your child must have:
- Diphtheria, Tetanus, & acellular Pertussis (DTap) – 5 doses*
- Polio (IPV) – 4 doses**
- Measles, Mumps, & Rubella (MMR) – 2 doses
- Hepatitis B (HepB) – 3 doses
- Varicella (VZV) – 2 doses
